چکیده

Abstract The prevalence of atrial fibrillation (AF) in end stage kidney disease (ESKD) patients undergoing dialysis is high, however, the high risk bleeding often hampers with a correct anticoagulation ESKD AF, despite thromboembolic risk. Left appendage (LAA) occlusion possible alternative to oral (OAT) for thromboembolism prevention AF populations hemorrhagic purpose study was evaluate efficacy and safety LAA cohort procedure (LAA cohort, n = 106), comparison two other cohorts, one taking warfarin (Warfarin 114) without therapy (No-OAT 148). After median follow-up 4 years, Cox regression model, adjusted confounding factors, showed that hazard ratios (HRs) events were 0.19 (95%CI 0.04–0.96; p 0.045) 0.16 0.04–0.66; 0.011) as compared Warfarin No-OAT respectively. HR 0.37 0.16–0.83; 0.017) while there no significant differences between (HR 0.51; 95% 0.23–1.12; 0.094). Adjusted models lower mortality both 0.60; 95%CI 0.38–0.94; 0.027) no-OAT 0.52; 0.34–0.78; 0.002). Thromboembolic than expected according CHA2DS2VASc score (1.7 [95%CI 0.3–3.0] vs 6.7 per 100 person/years, < 0.001). In safe effective associated reduced OAT or therapy.

A Semi-Automated Algorithm for Segmentation of the Left Atrial Appendage Landing Zone: Application in Left Atrial Appendage Occlusion Procedures

Background: Mechanical occlusion of the Left atrial appendage (LAA) using a purpose-built device has emerged as an effective prophylactic treatment in patients with atrial fibrillation at risk of stroke and a contraindication for anticoagulation. A crucial step in procedural planning is the choice of the device size. This is currently based on the manual analysis of the “Device Landing Zone” fr...

Efficacy and Safety of the WATCHMAN Left Atrial Appendage System for Stroke Prevention in Chinese Patients with Nonvalvular Atrial Fibrillation: A Single-center, Prospective, Observational Study

BACKGROUND In patients with nonvalvular atrial fibrillation (NVAF), embolic stroke is thought to be associated with left atrial appendage (LAA) thrombi. The WATCHMAN LAA Occlusion Device has been shown to be noninferior to conventional oral anticoagulation with warfarin for stroke prevention in patients with NVAF. This study aimed to evaluate the procedural feasibility, safety and 12-month outc...

Percutaneous interventions in elderly patients with atrial fibrillation: left atrial ablation and left atrial appendage occlusion

Heart rhythm disturbances become of special importance when patients reach a senior age. Atrial fibrillation (AF) is the most prevalent arrhythmia among the old age groups, and its impact on morbidity and mortality becomes of paramount significance. In this population, AF is responsible for significant amount of thromboembolic cerebrovascular events, especially for disabling and fatal strokes. ...

A meta-analysis for efficacy and safety evaluation of transcatheter left atrial appendage occlusion in patients with nonvalvular atrial fibrillation

OBJECTIVES This meta-analysis was conducted to evaluate the efficacy and safety of transcatheter left atrial appendage (LAA) occlusion in patients with nonvalvular atrial fibrillation. METHODS The randomized controlled trials (RCT) or observational studies with any transcatheter LAA occlusion devices were searched in PubMed, Embase, and Cochrane library from inception to November 2015. The in...

Complex left atrial appendage morphology and left atrial appendage thrombus formation in patients with atrial fibrillation.

BACKGROUND In patients with atrial fibrillation (AF), most thrombus forms in the left atrial appendage (LAA). However, the relation of LAA morphology with LAA thrombus is unknown. METHODS AND RESULTS We prospectively enrolled 633 consecutive patients who were candidates for catheter ablation for symptomatic drug-resistant AF. Transesophageal echocardiography (TEE) was performed to assess LAA ...
